Appointment Scheduling Details
When contacting the office to schedule an appointment, please have your referral information and insurance information available, as this will assist us with scheduling your appointment as promptly and conveniently as possible, and in providing you an estimate of possible treatment costs. If you are experiencing discomfort or have a dental emergency, please inform us when calling, as we will make every attempt possible to accommodate you the same day.
Payment Options
Prior to initiating treatment, we will provide you an estimate of your financial responsibility for treatment. Our fees are competitive for this region and represent the quality of care we provide. Payment will be collected at the start of treatment. For your convenience we accept Visa, MasterCard, Discover, American Express, and cash. We do NOT accept personal checks. Additionally, we offer financing through CareCredit ®.
If we are a participating provider with your dental insurance plan, we will provide you an estimate of your financial responsibility, based on the current insurance coverage information available to us. We will collect payment for deductibles, co-pays and charges for non-covered services at the start of treatment. If we are not a participating provider with your dental insurance plan, we will collect payment for all treatment charges at the start of treatment. As a courtesy, if you provide complete insurance information at the time of service, we will submit a dental claim for your direct reimbursement.

We are currently specialty providers for the following insurance carriers:
- Aetna - PPO & FEDVIP plans
- Delta - Premier & Preferred PPO & FEDVIP plans
- Guardian - Dental Guard Preferred PPO & some Preferred Select PPO plans
- MetLife - PPO & FEDVIP plans
- BCBS FEP - FEDVIP plans
Financing Options
If you would like to pay for treatment over time, we would like to introduce you to CareCredit® patient payment plans. CareCredit® provides a variety of flexible payment options, including interest-free payment plans. CareCredit® allows you to pay over time with convenient low monthly payments. Additional features include:

o Quick and easy application online or over the phone
o Receive a credit decision almost immediately
We currently participate with the 6 month and 12 month interest-free plans, and the extended plans with interest. If you would like to pay for treatment using CareCredit®, please apply prior to your appointment.
